How did everybody else do this? Is it just a matter of obscene amounts
of
repetition - of for example second space A to third line B transitions
and
then various combinations etc until it "just happens" or is there a
set of
logical rules to follow and a methodology to boot etc?

The big thing that most students do wrong is that the 3rd finger (the
one you play for the note D) of the right hand tends to go up too far.
Up meaning that the finger rides towards the middle finger - you have
to spread the fingers further apart and the D will seal properly if it
is ridding up when playing the B. Also make sure to have the C key
down with the right pinky as it will help you as well to position the
fingers properly for the break cross. Don't be in the least worried
about letting the 2nd space note "A" key go as you can actually play
the B with the A key pressed! (same for the 2nd line G#).

Practice getting the low E and then pressing the register key to go to
the next register.

Avrahm Galpers "Clarinet Method" book 1 is really good for the
exercizes to do just that - written by one of the very top masters of
the Clarinet world. Luyben Music has it 816-753-7111 is their number
(it's in the USA).
