I have seen this mechanism before, on some older Leblanc clarinets, I believe. It is supposed to allow you to trill or tremelo from C or C# to D# (Eb) by lifting the middle finger only.

On the ones I tested I found both the tone and the intonation lacking. I would rate it as an unsuccessful aattempt to fix a problem area on the clarinet.
