They do indeed have a form. They ask for very explicit information about the
instrument so they can do some version of carbon-dating it. I filled out the
form two weeks ago and so far have heard nothing. I am surely missing something
but I cannot understand why, given the detail I had to give them, it has taken
this long to hear not a word.
