Mouthpieces definitely CAN make a BIG difference. I have not found a
Vandoren mouthpiece yet (B44, 2 B45's, B46, 5RV tried) that would play in
tune in the altissimo on my Selmer Series 10 or most of my other clarinets,
yet various mouthpieces by Selmer, Leblanc, O'Brien, etc. work fine. On
the other hand, I have an old beat up Selmer BT that has a horrendously
SHARP altissimo register otherwise, but plays perfectly and effortlessly in
tune with a B46 or B45.

Of course, you are absolutely correct that a previously functional setup
that is now NOT working can be caused by mechanical problems with the
instrument. Removing that factor from the equation is an important first
step in the process.
