Jim,
I`ll second the sports injuries specialist/clinic possible solution, but as
only <one> of several directions you should try for. Find a good one.

But have you already seen a specialist? A <pain> specialist doesn`t sound
quite right to me. You may need to see the right kind of specialist for your
original complaint, to maybe discuss the possibility of an operation. This
way forward sounds more logical to me than just trying to manage the pain
only.
