They should make a clarinet (or a bari sax) rigged up somehow for diabetics.
When you need to dose you just lean on a needle spring. I guess you could
do it for heroin addicts, too, if one was so inclined. :)

I used to play a bari sax that had a needle spring that projected about 1/8"
beyond the post it connected to -- if I grabbed the horn in such a way that
my fingers slid under that post, I could count on having about 1/8" of that
needle jabbed in my finger. Happened about every 3 months...
