Soak the reed in Efferdent or Polident, whichever you prefer.
Soak in a cup, like you would a set of dentures, but only soak
until the effervescence is gone, say 15-20 minutes. After this
is done, take the reeds out, and push the solution out of the
reeds with your fingers starting from the bottom cut of the reed
toward the tip (NEVER the other way or you will destroy the
reed.)

I have found this not only works on mildew, but it works on
renewing a reed for a few more plays when a particularly good
one wears out, or when it gets a little dirtier than I can clean
by hand alone. Seems to clean gunk in between fibers and
facilitates the reed fibers in bonding back together into a
cohesive form.

I don't remember who told me this, but I do remember it was a
while back in college. It has always worked for me though.
