I'm not sure this is an accident, but it was pretty funny.
In HS eons ago, I played one season in the city's Philharmonic Youth
Orchestra. Behind me was the trombone section and there was one player who
was somewhat of a show boater who always managed a flashy long slide out
and back after he played. It was distracting because the slide went past
the side of my head.

During an evening performance, for the grown ups....g, we were playing
something or another and suddenly this trombone slide shoots out, bangs on
my music stand flips over and slides across the stage coming to rest
directly in front of "the regular Philharmonic's conductor", not the youth
conductor. To us, this was a fearsome individual. We finished the first
half of the program, took our bows and the conductor picks up the slide,
walks over to the trombonist, handing it to him.

I heard the conductor say to the fellow, "we will not do that again will
we?" The entire audience was giggling so for the rest of the season, no
more flashing slide!
