Certainly. The C# can be played with the LH first finger down and the
thumb up, but the register key still depressed. Lowering the thumb then
gives the B - both are in the clarion register. Whether the C# is in tune
or not depends on the clarinet, but since you say these are fast notes, it
may not make much difference. It takes some practice to wiggle the thumb
this way, but it can be done.

Don't be afraid to experiment with odd combinations. For instance, another
C# is the usual C plus the lower two trill keys, which also works for the
F# in the lower register. Not much use for what you want, but useful in
other conditions. Etc, etc.
