If I remember correctly, Howard Hansen Symphony #2 has a contrabass
clarinet. I played it on a bass clarinet, though.

[Jeroen T. Salm] I remember me playing a huge (wooden) Selmer
Contrabasclarinet in the Concertgebouw Amsterdam in the piece The Flood =
by
Igor Strawinsky.
I will always remember it, because I had to hold that instrument steady =
in
my hands all the time, because there was (then) nothing available to put =
the
instrument in when not playing it. It's a rather long and heavy =
instrument
and a ditto work by Strawinksy: the next day my muscles and back ached
terribly... It was a monstrous instrument, haha, but what a sound, and =
what
a huge reed!
