Ian,

Just a few thoughts.

Reed is too soft
Clarinet has a leak
Embouchure and or clarinet relationship to player is incorrect....(overbite
is an advantage by the way)
flex of bottom lip
biting
no support of clarinet from right hand thumb up and into top teeth
Tongue position or "voicing" is incorrect
Improper start of sound

Have you tried her set up?
Has she tried another clarinet/mouthpiece/reed combination?
Even in a beginning student....teach them to start the sound properly with
the tongue....helps this kind of problem
