Some time back there was discussion of air leakage inside the throat area.
I was going through some of my papers and came across this item from the
Southern Medical Journal, 9/6/02. There was a discussion of otolaryngologic
disorders, among which was "velopharyngeal incompetence".

A 17 year old wind player had velopharyngeal incompetence while attending
band camp, where he practiced 8-10 hours per day, approximately 4 times his
usual practice duration. Nasal air escape developed during instrument play,
as well as nasal regurgitation of liquids and hyper nasal speech. Rest and
palatal exercises were prescribed and his condition improved.

I don't know if this info is helpful to anyone, but I thought I'd pass it
along.
