In crossing the top break -- for example, going from B to D -- the
technique is to slide the LH first finger progressively downwards over
the hole, uncovering it gradually during the transition, so that the
hole isn't necessarily completely uncovered when the other LH and RH
fingers arrive at their positions for the D.
