The SHAPE booklet (to which someone referred earlier, I wrote off for a
copy) mentions both loss of high hearing and diplacusis, but it refers
to them as separate problems.

The booklet says that diplacusis is sometimes a difference in _time_
perception between the two ears and other times a difference in _pitch_
perception. The booklet doesn't give further details.
