Mix white (unflavored) vinegar with water --- 1/4 vinegar to 3/4 water.
Allow the entire mouthpiece to soak for 10 minutes or so.

The one thing you should *not* do is drag a swab forcibly through the
mouthpiece. @-----. =A0 You want to protect the sharp
edges of the mouthpiece's tip and rails and table.

If there is enough 'gook' in the mouthpiece to make you worry about
sanitation, there are mouthpiece disinfectants; but read the labels
carefully. @-----. =A0 One
disinfectant whose label claims not to attack hard rubber (I use it
myself) is "Sterisol Concentrate for Wind Instruments."
