I think that it is fairly common, especially in the examples noted, that
players switch instruments. Many trumpet players I know do it all the time,
to say nothing of horn players. Is it possible that due to the frequency of
this, that this has actually happened to more than one player over the
years?

I once had an experience that goes in the opposite. I was playing a piece
where the first part is on Eb and the second on Bb. I was playing along on
the Eb when the conductor stopped the orchestra and bellowed "first
clarinet! Isn't that supposed to be played on Eb?" I nodded and proceeded to
raise the instrument above my head so he could clearly see that it was an
Eb. He looked flustered and after a short pause said "Well, could you make
it sound MORE like an Eb?!"

Imagine that! I actually was reprimanded for playing what was in the part.
