When I was particularly unsatisfied with my inability to solve the sort
of problems you have described, I switched from Vandoren's B-40 series
to Clark Fobes' Nova mouthpiece (on advice from my teacher). Clark has
a web page, and he's a Klarinet sponsor. You'll find a link on
Klarinet's "Sponsors" page.

The Nova is not one of Clark's "artist level" mouthpieces, but it
_*does*_ solve problems for people who (like me) are still learning.
At least, it did for me. They're not especially expensive, either. I
still have the B-series mouthpieces in my drawer and I try to play them
occasionally. I still have the same problems with them.
