Dan, all sorts of glasses can be made. Looks like what you need is plain
glass for the bottom 3/4 or so, and your distance correction ( would be a
concave lens) at the top. Speak to your glasses supplier - I'm sure such a
thing could be made. I have no idea what it would cost, but probably not
much different from standard bifocals.

(BTW, I once knew an electrician who had to work close up and overhead, and
had a set of glasses made with a close-up strip along the top of the lens
in addition to the usual "bifocal" one at the bottom. He said it worked
great.)
