Possible answere:

Composers don't always get it right. A composition is usually the
ultimate gedankenexperiment. Composing at the piano could cause the
problem - or composing while walking in the woods - or the instruments of
Beethoven's day might well have been more flexible. (I favor that concept
because the instruments I've heard seem to be veeerrry flexible.)
