I like the idea: 'clarinetto ripieno', like 'panino ripieno'.

(A panino ripieno is an Italian sandwich stuffed full of goodies.)

So perhaps my Eb clarinet with chicken entrails down it would be
'clarinetto ripieno':

In music, though, 'ripieno' is, for example, the bit of an orchestra
which supports the more soloistic 'concertino' in a concerto grosso.
It's the 'filling', but unlike in a sandwich, it's the *less* important
bit.

So I think he means that mostly the clarinets don't have very
interesting parts.
