How many questions? How much time should I expect to complete the
survey? Will the survey be designed by someone experienced in
information design, so that I don't feel compelled to write comments
next to every question to clarify what I really mean? (Sorry, I've spent
too much time filling out poorly-designed surveys.)

To me, the answers to those questions are more important than the medium
used to transmit the questions and collect the data.
