Very simply, a neckstrap is to help support the weight of the clarinet. The
thumb is really not a good design to support the weight and so some people
have difficulties (pinched nerves, pain, etc) that can be alleviated or
prevented by using a neckstrap. There are even some oboists who use
neckstraps although this is even rarer than clarinettists using them.
