Depends a lot on the context. One that's used often is regular Bb (thumb,
register key, 1st and 2nd on the left and right hand side key) and for D
pick all of your left hand fingers up (squeak an open "G" but you can leave
the right hand side key down). The D will be a little out of tune, but the
effect is very smooth and it's easy to do.
