Play the Bb above the staff as thumb, register key, 1st left hand finger and
1st right hand finger.

For the D, just lift the the 1st left hand finger and put down the 2nd and
3rd left hand fingers. Since a tremolo is quick anyway, omit right hand
little finger on the Eb/Ab lever. Trying to get that lever makes the
tremolo too awkward.
