I bought one of the less expensive Kooiman "Etude" thumbrests just this
week. In the UK this model costs £20 compared to £100 for the pro model. The
principle is the same with the weight on the thumb closer to the palm.

This model is less adjustable than the pro model but you can fit it yourself
using the existing two thumbrest holes (at least on a Buffet RC you can). It
took me about 15 minutes. Its too early to say if it will help my arm and
shoulder pains but my initial trials have been promising.

There are more details on both models on the website www.rokatec.com

I got mine from Muncy, who did the initial installation on my backup Bb
(to see how I liked it)

When I decided to make it permanent, I was on my way to my regular tech,
Mark Jacobi, and he did the installation. As Bill points out, it is not
just a replacement for the regular thumb rest. It needs three screws
which must be pre-drilled in different locations from the regular thumb
rest screws. Just watching it get done made me really nervous!

I got the top of the line one, which is more adjustable, if I understand
correctly.
