About 6 weeks ago I had the Kooiman thumb rest mounted on both my
clarinets. It transfers the weight of the clarinet to behind the second
joint of the thumb, and frees up the RH fingers incredibly. It's
infinitely adjustable in pretty much any direction with a series of
joints tightened with a hex wrench, to suit any hand shape and size.

I was all set to have it removed from my Bb clarinet, where I'd installed
it to try it out, when I took it to show a physical therapist friend who
was looking to help another clarinetist. She showed me that I had it set
up wrong, and once it was fixed it felt great.

The usual disclaimer - I have no financial or other interest in this
device - just a satisfied user. BTW, the PT said it was the most elegant
solution to this problem that she'd ever seen.
