Stewart,

As you're finding out, breaking old, deeply entrenched habits is very, very
hard to do. About the only things I venture to suggest would be to set aside a
part of every practice session to concentrate on this issue. If you think about
it every day it should gradually start coming around. Another suggestion would
be to do this practicing in front of a mirror so that you can observe your
fingers' shape and position when playing both slow and fast moving sections of
notes. If you were to add up all the total hours of time you have done it the
old way, it would be a huge amount of time. Be patient, yet persistent with
yourself.
