Interesting discussion. Don't forget Tom Ridenaur's book(s)--especially the
one about altissimo fingerings. Bill Smith's Variants, which has been around
for a good thirty years now, goes way up in the A neighborhood, 8 ledger
lines above the staff. And it can be done, though many folks just put their
teeth on the reed for that one.

I think the answer is, you can play as high as you can play.
