Every clarinet player needs a complete fingering book which answers
this question (and many others).

The book that I like costs about $15 (?), has very clear diagrams,
and fits easily in a shirt pocket. Its title is "303 Clarinet
Fingerings & 276 Trills". Among other places, you can buy a copy from
Gary Van Cott (who posts on this list) at www.vcisinc.com.

By the way, the book says that one of the preferred fingerings for
__G an octave above 4 leger lines above the staff__ is the thumb and
register key all by themselves. This is also a preferred fingering for
C that is only two leger lines above the staff, which everybody needs to
play eventually. I can't approach the octava G myself, but the
difference between these two notes is embouchure and tongue position and
breath support, not fingering.
