My own self inflicted horror story came when on bass clarinet in "Kiss me
Kate". (Does the bass attract these horrors?). The introduction to the solo
voice is but one whole note cue given by the bass. I turned over two pages
and played <that> intro, which was a whole bunch of descending 1/4 notes.
The whole orchestra collapsed into fits of laughter - - - -I panicked, and
was almost ready to play it again when my 1st player noticed my wrong page.
The <singer> panicked for the rest of the run.

Another experience of pushing the high rise chair back too far happened not
to me, but to Duncan Campbell, one of the trumpet players in the Ted Heath
Band. Warming up just before the concert started, he just disappeared
backwards, behind the backcloth curtain, chair and all, in a clatter of
bangs, yelps and trumpet splutters. He wasn`t hurt. Nor was his trumpet, but
it`s hilarious to watch it happen.
