One book says that the same graphic for a one-note tremolo is
measured if the graphic stands alone, but it is unmeasured if the
letters "trem" appear above the graphic. This seems artificial to me.

As another example, I found disagreement whether a beam and two
free-floating bars in a measured tremolo of two notes means 1/16 (the
beam doesn't count towards the subdivision) or 1/32 (the beam does
count).
