For starters, this is not a trill but a tremolo. Generally it should be
played at a certain speed as dictated by the tempo of the piece and the
number of bars connecting the two notes of the tremolo (if written as two
half notes with bars connecting the stems). One bar means play as eight
notes, two bars as sixteenth notes, etc. So it may be possible that
difficulties are arising from trying to play it too fast.

To alternate between the throat Bb and the D above, it is generally
effective to simply keep the right hand fingers in position at all times.
That is to say, actually on the holes. Then you only have to move the left
hand fingers. The Bb should be played as A key plus register key and the D
as the normal fingering.
