Ed is quite correct, but in the event that this restriction applies, the
payment of an additional fee with negate it. Insurance companies will
do anything for an additional payment and I think that they base their
rate structures on the hypothesis that if they include everything at one
price, that is a tactical and strategic error.

It has given rise to jokes such as "My insurance policy only covers me
if I am eaten alive by a buffalo in the Brooklyn Battery Tunnel on an
odd Tuesday of any month."
