I always swab from the bell to the top. That way, the water doesn't get
pulled through the rest of the horn, and it's just so much easier to get the
dangly thingy into the bell than into the barrel. I've never had my (silk)
swab get jammed; I just always make sure that it's not all bunched up. My
cousin, however, who is a bassoonist, got her swab stuck in the bocal so
tight that she had send it to a repairman!

From the mouthpiece (which has the smaller diameter) towards the
bell (which has the larger diameter) because this avoids the possibility
of getting the swab jammed inside the instrument.
