Two possibilities come immediately to mind:

a) dirt blocking the register tube (or, if it happens only after you've been
plating a few minutes, a little dust plus enough condensation to block the
hole). - Try removing the register key and cleaning the tube with a pipe
cleaner or a paper clip.

b) thumb is coming off the thumb hole as you press the register key. This
can happen if the register key is too far from the thumb hole, too high, or
both. It is most likely if the notes will play sometimes (when you go down
toward them or start them cold) but not other times (when you go up over the
break to them.

Neither of these will likely explain B-C-D _never_ speaking. That's more
likely a reed/mouthpiece problem coupled with too much embouchure pressure
(the reed may be closing, which for some reason tends to cause more problems
with the long notes of the "clarion" register than others).

If you can't track it down yourself, ask another player to try it (with
his/her mouthpiece/reed) and see if he/she has the same trouble.
