Yes you measure the total length of the barrel. The 66mm is standard for
many clarinets but not all. I know of at least one older clarinet that was
designed with a shorter upper joint so the barrel was designed longer to
make the overall length correct.

Differences in diameter and shape can change pitch.

Some people choose to get a barrel from an independent maker as they are
dissatisfied with the stock barrel.
