It's possible that this novel was never published outside the USA. I looked
on Bookfinder:

http://www.bookfinder.com/

and on Abe Books

http://www.abebooks.com/

It's not a commonplace book -- evidently the print run was small -- but Abe
lists a dozen copies, and bookfinder more than a dozen, at prices ranging
from US$7.50 to about US$35.00. All are 1992 St. Martin's first editions.
(I believe there was no second edition in hardback.) All the copies listed
were located in the USA. AFAIK, the book is not greatly in demand, so I'd
recommend paying less than US$10, unless it's signed or fine with a dust
jacket. It should be selling as a used book, not as a collectible. These
sites don't list paperbacks, except for historic rarities. I don't know
whether _Fantastique_ came out in paperback, but I'd be astonished if it
didn't, even with the poor hardback sales.

Good luck with the replacement clarinet. That section of "Symphonie
Fantastique" sounds tricky to play, even without the instrument packing up in
mid-rehearsal.
