Dan,
1963 was the first issuance of the ASCII standard; since the term wasn't
"standardized" before that it, of course, could have meant anything!

The standard was proposed to the ASA (the forerunner of ANSI) in 1961 by Bob
Bemer who at that moment in time worked for IBM. Most references say that
ANSI adopted it, but since ANSI only dates from 1969 those references are
technically incorrect. It was blessed as Standard X3.4 in 1963.
