I've just recovered from a similar problem myself. Sometimes a
person simply doesn't realize how severely he or she is altering
embouchure or breath when attacking a certain note or jumping a certain
interval. A big change of tone while crossing the break is an obvious
example, but it happens elsewhere as well. I just learned that from my
own experience.
Sometimes you need a teacher to show you exactly what's happening
(perhaps by example), or sometimes you can feel it yourself if you focus
your attention or if you watch yourself in a mirror.
