1) The pressure we as woodwind players maintain is quite a bit lower than
the pressure the brass players maintain (most, if not all, of those reports
were of brass players) - even during a long concerto, we have enough rests
etc... that I don't think there's any danger, unless you have one of a
series of specific anatomical anamolies.

2) Even during a concerto, we really don't maintain the pressures for all
that long - we still take regular breaths.

Yes, but I knew that they were referring to the one in the heart because
they used the term "patent foramen ovale". That is the medical term for
the failure of the one in the heart to close...
