Similar and even easier to find--round pencil grip sleeves (this isn't
the official name, i have thrown out the original package), available in
office supply store as well as Wal-Mart and K-Mart (for U.S. folks).
Tube of foam, 1.5 inches long.
can be cut into three thumb rests.
Comes in a variety of interesting colors...

FWIW, i have the standard thumb rest on Bb (with the custom (grin) padded
cover and a Bay adjustable (no longer made?) on the A. By the picture in
various catalogs, the Kooiman is similar. Anyway, the plate is larger and
the height and angle are adjustable.
