This x-ray is called a Panorex (panoramic x-ray) and is not new, havung been
available for the last 20 years or so. There are skeletal issues that
cannot be adequately seen in this picture. This is where the oral and
maxillofacial surgeon is needed.

Karl's suggestion of sinus examination and x-rays is very good. This problem
should be ruled out before proceeding with root canal therapy.
