When I had similar problems, I misdiagnosed the problem and I
thought that I needed a fully adjustable thumb rest. It turned out that
what I needed was one of those simple rubber cushions that most music
stores have on the shelf -- $5 or so.
My tightness problem was more related to the way that I was
subconsciously tensing my fingers because the metal thumb rest was
digging into the flesh of my thumb.
However, the cushion is also thick enough to move my thumb closer
to the holes, and I think that this helped as well. The cushion that I
purchased is thicker on one side than the other: 3/8" (10 mm?) on one
side and 3/16" (5 mm?) on the other side. This means that I can flip
the cushion in order to adjust the distance a little bit. The thicker
side works best for me.
