For about $15, you can buy a fits-in-your-shirt-pocket book called
"303 Clarinet Fingerings & 276 Trills" by Alan Sim. Even though this
book is pocket-sized, I find it easier to read (and carry) and
definitely more complete than the other fingering charts that I've seen.
It tells you the strengths and faults of various fingerings, as well as
showing how to do them.
Among other places, you can buy this book from Gary Van Cott
(www.vcisinc.com).

For the slur, I'm guessing that you mean three lines and a space
above the staff?
The book shows L123 + R123 + left C# + thumb + register key for the
F and then lift R3 for the F#.
I can't get this high myself, so I've never tried it.
