Rehfeldt's book is a book of _very_ specialized fingering (and leggings :^).
The legs, thighs, knees, etc. are used to cover holes or press keys that
cannot be pressed using the fingers since the fingers are already being used
in some other combination. These very specialized fingerings are for
microtonal adjustments and coloring, something that you may never have to
use - there are few pieces which require this kind of thing.
