Careful inspection, visual or otherwise, cannot find everything. If the
flaw happens to lie where you do not cut across it during the manufacturing
process, visual inspection certainly won't find it. You spoke of magnetic
imaging, ultrasound, etc. Well guess what. They don't always work either.
It is very difficult to interpret the images accurately with 100%
reliability. Flawed parts can still get through these inspection
techniques.

It has not yet proven possible to make 100% of all metal parts, where there
is much more control over the raw stock, with absolutely no flaws so how can
it be possible to produce perfect wooden parts, a raw material with
substantially more variation. Year by year, manufacturers of metal parts do
reduce the return rate but warranty returns *still* occur.

Perhaps someday the world will be perfect and all goods and services will be
perfect but I'm not going to hold my breath waiting for it or get all bent
out of shape when I find I need to use the warranty policy on something I
have purchased.
