I know the problem well - have had the same problem for ages - My instructor
has me subdivide absolutely every thing - As an adult learner, I had many
years of bad habits to unlearn.

Try playing the sixteenth notes as 1 beat. When I now look at a new piece, I
play the smallest note as one beat to start. That means I may hold a quarter
note for 4 beats at first. But I play each beat in the quarter notes - It
is slow going at first. Once I can play the notes in time, I then increase
the speed. I have found good slow practice makes for for less time on the
piece in the long run. I am now to the point I will not sight read a tricky
section, but skip it until I can take it home and learn it without mistakes.
If I don't make a mistake at the slow speed, I can learn it quicker for the
higher speed.
