This is the title of a survey conducted by Cecil Gold
to which he was sent a total of 162 replies.
Respondents are listed in Appendix E
If they are required I will try scanning them

Questions 8 and 9 out of 22 refer to mouthpieces

I am guessing, but it might be something developed from this
