This varies from what you describe, the change i identify is in raising
the soft palate.
i discovered this from voice lessons, where i learned to be aware and
control of the soft palate, then realized the sensation of the raised
soft palate was familiar for smooth register connections on clarinet.

i don't know if we are doing the same thing by different sensations OR
because of different structures make differing adjustments.

As i wrote in reply to Nicholas, i have worked from the pianissimo end,
when specifically aware of grunts. Either works.

Also i just checked, and the grunts are also apparent in register slurs,
especially when tongueing the clarion note. This is where i begin the
refining for me and students.

Also helpful that i can intentionally sustain a grunt/ghost sound for
demonstration for students. Do it on purpose, then not, clarifies the
differences.
