BAYSIDE MUSIC PRESS ORDER FORM

Here is the order form to accompany your new Bayside
Music Press catalog. To print it out, please do the
following:

(1) Read items 1-6 on this list. Remember what #5 &
#6 say, because we're going to delete them in step 4.

(2) Copy and paste the entire text of this order form
into any program that allows you to save it as a text
(*.txt) file. (In Windows, you can use Notepad.)

(3) Set your top and bottom print margins to one inch.

(4) Delete everything from the top of the file down
to and including the row of asterisks (*****).

(5) Save the file under a name you will remember.

(6) Print it out. It should be two pages long, and
the page break should come out in a sensible place.
If it doesn't, don't worry about it -- just place
your order (hint!) and we'll figure things out when
it arrives here.

(1) E-mail your order, and then telephone your credit
card information to (408)-848-8097. If we are out, you
may safely leave your information on our double password
protected voicemail system.

(2) E-mail your credit card information, giving verbal
equivalents for all numbers. For example, a card number
of 1234-5678 would become "one-two-three-four, five-six-
seven-eight." An expiration date of 12/99 would become
"December 1999." Be sure to tell us which card - we
accept Visa, Master Card, and American Express.

Credit card orders will be processed through NORTHWEST MUSIC
SHOP, DAVENPORT, IOWA, and will appear on your statement as
a purchase from Northwest Music Shop. By law, Credit Card
orders delivered to an Iowa addresses must be charged the
